Promote the "integration of medical and prevention" chronic disease management model

Recently, Jia Weiping, an academician of the CAE Member and former president of the Sixth People's Hospital of Shanghai, at Sancai Health Center in Lingshui Li Autonomous County, Hainan Province, carefully looked through a stack of chronic disease management files, and asked the head of the hospital in detail. Such visits and research are part of Jia Weiping's daily routine. For more than 40 years, Jia Weiping has always taken root in the research and prevention of diabetes. She led the team not only to build a scientific system for the diagnosis and treatment of Chinese diabetes, but also to innovate and create a regional chronic disease governance model of "integration of medicine and prevention" with Shanghai as the fulcrum, so as to inject lasting impetus into the high-quality development of China's medical and health undertakings. As a scientific researcher, Jia Weiping, guided by solving clinical problems, has achieved many "firsts": for the first time, 12 new susceptibility genes for type 2 diabetes in Chinese have been found, and a genetic risk early warning model has been constructed; It is the first time to establish a reference value for the clinical application of continuous glucose monitoring, which has promoted the widespread application of this technology... As a clinician, Jia Weiping is saddened that many patients often have serious complications of diabetes when they come to the tertiary hospitals for medical treatment. Jia Wei usually said that diabetes has a long pathogenic process, many predisposing factors and covers a wide range of people. If we only focus on "treatment", we can only "get half the result with twice the effort". Only by forming an integrated system engineering management strategy of disease prevention, accurate diagnosis and treatment, and scientific management, is the key to the prevention and control of diabetes. This requires early detection and diagnosis of diabetes, and moving from individual diagnosis to group prevention and control. However, after multiple on-site investigations at grassroots community hospitals, Jia Weiping found that ideal blood sugar control can reach over 60% in tertiary hospitals, while this proportion is only 20% -30% in community hospitals, and community hospitals have not conducted complication examinations for many years. Since 2007, under the leadership of Jia Weiping, Shanghai has pioneered the "hospital community diabetes integration" management model in China, turning the specialized resources of diabetes in tertiary hospitals into suitable technology that can be popularized, sinking into the community, and realizing the seamless connection of community first diagnosis, two-way referral, and quick and slow treatment. Anil Kapp, President of the World diabetes Foundation, praised this as the "Shanghai model" for diabetes prevention and control. The "diabetes health cabin" of Caoyang Community Health Service Center in Putuo District, Shanghai is the epitome of the "Shanghai model". Here, the archives management of diabetes patients and high-risk groups has been established. Through appropriate technology, diabetes chronic complications screening has been carried out for patients in communities, and the "hospital community" sharing information platform has been developed, forming a systematic management link with the Endocrine Metabolism Department of Shanghai Sixth People's Hospital. Today, through the construction of diabetes prevention and treatment service system in Shanghai presided over by Jia Weiping, this model has been extended to more than 240 communities in Shanghai, providing residents with diabetes full service integrating health education, disease screening, follow-up management and self-management support. Jia Weiping is even more pleased to see that the "Shanghai model" has been implemented nationwide and successfully replicated in more than 20 provinces and cities, promoting the breakthrough of grassroots chronic disease prevention and control. The experience of educated youth made Jia Weiping pay special attention to the prevention and control of diabetes in rural areas. She keenly perceived and foresaw that the improvement of living environment conditions would lead to changes in the 'disease spectrum' in rural areas. In 2014, with this topic in mind, Jia Weiping came to Jingyuan County, Guyuan City, Ningxia Hui Autonomous Region. "We were surprised to find that despite our remote location, our concern has already occurred. The prevalence of diabetes in the local area has significantly exceeded the national level, with 1/3 of the people being overweight and obese, and chronic diseases such as thyroid disease and atherosclerosis have a high incidence," she said. At the same time, another problem also worried Jia Weiping: the rural health centers are short of drugs, and there are only two or three commonly used drugs to treat hypertension and diabetes. If these two or three drugs are used properly, they can already solve 70% to 80% of chronic diseases, "she said. However, rural health centers in remote areas generally lack medical personnel with general diagnosis and treatment skills." The lack of medical knowledge often prevents them from fully utilizing these drugs, "Jia Weiping began to replicate the successful training experience in Shanghai communities. Jia Weiping, who once had the experience of "going up to the mountains and down to the countryside" in Shaanxi, runs a study class in Jingyuan County, providing health knowledge training to local residents in Shaanxi dialect and training rural doctors on rational drug use. She also invited medical staff from Hongsibao District People's Hospital in Wuzhong City to study at Shanghai Sixth People's Hospital, which focuses on targeted poverty alleviation, to help them formulate disease prevention and control plans according to local conditions. Through solid on-site visits and research, Jia Weiping has a solid basis for providing policy recommendations to the decision-making level. In recent years, Jia Weiping has led nearly 20 national projects and major projects in Shanghai, promoting diabetes research in Shanghai to become a national highland. She led the development of more than 30 national and industry guidelines, providing standardized diagnosis and treatment basis for local medical institutions. With the support of projects such as the "Supply side Reform Research on Chronic Disease Prevention and Control and National Health Management" by the Chinese Academy of Engineering and the "Development Strategy Research on Future Pharmaceutical Industry and Future Health Industry" in Shanghai, Jia Weiping has contributed many insightful views to the development of chronic disease prevention and control and pharmaceutical health industry in Shanghai and across the country.
