Recently, six departments including the National Health Commission, National Medical Security Administration, and Ministry of Finance issued the "Guiding Opinions on Strengthening Grassroots Chronic Disease Health Management Services" (hereinafter referred to as the "Guiding Opinions"), leveraging the advantages of grassroots medical and health institutions in disease prevention, treatment, and health management to strengthen the full process health management services for chronic diseases at the grassroots level. The Guiding Opinions clearly state that we must adhere to the combination of prevention and treatment, give equal importance to traditional Chinese and Western medicine, give full play to the role of close medical alliances and family doctor contract services, integrate the health management service functions of grassroots chronic disease prevention, screening, diagnosis, treatment, management, and rehabilitation in conjunction with basic public health service projects, provide comprehensive, systematic, and continuous services for the masses, and improve the quality and effectiveness of grassroots chronic disease health services.? The Guiding Opinions require that the roles of the four types of institutions be fully utilized. One is to leverage the role of township health centers and community health service centers as hubs for chronic disease health management services at the grassroots level, to carry out chronic disease prevention, diagnosis, treatment, and health management services, as well as to connect with referral services. The second is to leverage the fundamental role of village clinics (community health service stations) in chronic disease health management services, carry out health education, evaluate, follow up, classify intervention guidance, health counseling, etc. for chronic disease patients, and guide referral when necessary. The third is to leverage the leading role of closely knit medical consortia or higher-level hospitals to strengthen the management of patients with chronic disease crises and serious complications, and guide and support grassroots work. The fourth is to give full play to the technical guidance role of professional public health institutions such as disease prevention and control centers. The Guiding Opinions put forward multiple safeguard measures. Specifically, it includes: strengthening the medication guarantee for chronic diseases at the grassroots level; Strengthening support measures such as training grassroots personnel and improving service capabilities; Strengthen the empowerment of digital intelligence, promote information exchange and sharing among institutions, and promote the grassroots application of medical artificial intelligence assisted technology; Strengthen quality control, consolidate the quality control responsibilities of relevant hospitals and grassroots medical and health institutions, and work together to improve service quality; Strengthen work implementation, improve work mechanisms, closely coordinate departments, and form a working force.
