Shanghai takes the lead in establishing a fetal hospital to promote the integrated development of obstetrics and pediatrics

2025-01-25

On the 24th, it was learned that in order to fill the disciplinary gap between obstetrics and pediatrics, Xinhua Hospital affiliated with Shanghai Jiao Tong University School of Medicine (referred to as Xinhua Hospital) has officially established a fetal hospital, breaking down disciplinary barriers, promoting the deep integration of "fetal medicine" and "intrauterine pediatrics", and opening up a new development pattern for maternal and child health. Professor Sun Kun, Chairman of the Pediatric Branch of the Chinese Medical Association, President of Xinhua Hospital, and President of the Fetal Hospital of Xinhua Hospital, stated on the same day that the establishment of a fetal hospital is to further focus on early life health from fertilized eggs to two years after birth, carry out multidisciplinary diagnosis, treatment, and research on fetal diseases, promote the integrated development of obstetrics and pediatrics, and take the lead in establishing an original plan for sequential diagnosis and treatment of "fetus newborn child". At the 2024 Academic Annual Meeting of the Fetal and Intrauterine Pediatric Medicine Specialty Branch of the Shanghai Medical Association held on the 24th, it was learned that the proposal and clinical practice of fetal medicine as a sub discipline of obstetrics in China originated in Shanghai. In November 2009, Professor Sun Luming, who had received formal clinical training in maternal and fetal medicine abroad, opened the first clinic in China dedicated to treating fetuses - the "one-stop clinic for fetal medicine"; In February 2010, Professor Duan Tao, a renowned expert in obstetrics, first proposed to divide obstetrics into three sub specialties: general obstetrics, maternal medicine, and fetal medicine, in the evaluation criteria for obstetrics clinical key specialties of the Ministry of Health (now the National Health Commission), laying the foundation for the legal and compliant development of fetal medicine in China. Intrauterine Pediatrics is an emerging discipline proposed and established by Xinhua Hospital based on the development of pediatrics, focusing on pediatric diseases originating from the uterus, including all fetal structures, growth and development abnormalities, and congenital organ dysfunction. In 2021, Dean Sun Kun first proposed the concept of "intrauterine pediatrics" with the aim of preventing and treating developmental diseases, focusing on early prevention, screening, diagnosis, and treatment of fetal diseases from the fertilized egg to the perinatal period and even the entire life cycle. This marks a further shift in the diagnosis and treatment platform of pediatric diseases to the fetal period. Although obstetrics and pediatrics both focus on fetal health, there are certain barriers between disciplines that limit the comprehensiveness and accuracy of fetal disease diagnosis and treatment. Obstetrics and pediatrics are taking a step forward, can we achieve 1+1 greater than 2? Fetal medicine doctors are the first and general practitioners of diseased fetuses, while intrauterine pediatricians are specialized doctors who diagnose and treat diseased fetuses on the fetal platform. Professor Sun Luming, Executive Director of the Fetal Hospital of Xinhua Hospital, revealed that facing the two "patients" of mother and fetus, both fetal medicine and intrauterine pediatrics urgently need to eliminate barriers between disciplines and hospitals. Guided by the core concept of "mother safety first, fetus is also a patient", a new clinical diagnosis and treatment model of "one-stop, sequential diagnosis and treatment" should be constructed. It is reported that the main body of Xinhua Hospital's fetal hospital consists of obstetrics, prenatal diagnosis center, fetal medicine and intrauterine pediatric disease diagnosis and treatment center, supplemented by the hospital's neonatology, reproductive medicine center, adult science, pediatrics and platform science, forming a disciplinary system for screening and treating fetal diseases. Within three months, Dean Sun Kun and Professor Sun Luming collaborated to push the integration of fetal medicine and intrauterine pediatrics to a new height: creating a multidisciplinary diagnosis and treatment platform to achieve deep collaboration among obstetrics, pediatrics, imaging, genetics, and other disciplines. This platform breaks the limitations of traditional diagnosis and treatment models, from screening and diagnosis of fetal diseases, to intrauterine intervention, postpartum treatment, and long-term follow-up, building a full lifecycle management system, truly safeguarding fetal health. Professor Sun Luming has established a specialized outpatient clinic for fetal medicine, which has treated over 300 cases of "fetal abnormalities". Dean Sun Kun revealed that the construction of the fetal hospital is based on the rapid development of fetal medicine in China and the creation of intrauterine and neonatal science. The implementation of this project will further enhance the overall level of maternal and child health services in Shanghai and even across the country, and make positive contributions to promoting the development of maternal and child health.
